Filling out an application for an old-style international passport

You can travel abroad using either a new or an old passport. If you intend to receive the second version of the document, you will need to fill out a standard application form for an old-style international passport.

The procedure for obtaining two types of passports is not much different.

Types of Russian foreign passports

Since you have the option of choosing the type of travel document, carefully consider the prospects you will gain by choosing one over the other. Today there are two types of travel documents for trips abroad:

  • biometric (new);
  • old style.

They will let you go abroad without any problems with both the first and second options. The only difference is that the new one has a chip with the owner’s personal data, which simplifies the procedure for passing passport control at the airport.

Many Russians, using their legal right to choose, specified in Article 8 of the Federal Law of the Russian Federation N 114, write an application to receive a familiar document without modifications.

If migration service employees find fault with your passport without biometrics, you can immediately file a complaint, since in this case the fault-finding is illegal.

Consulates and visa centers treat documents equally; no one has the right to refuse you a visa due to the absence of an electronic chip in your passport.

How to get an old-style passport

The first and most important thing to do is fill out an application for an old-style international passport. In 2021, a standard form is proposed, which can be downloaded on the official website of the Main Directorate for Migration Affairs of the Ministry of Internal Affairs.
You can also find a sample form on the Internet and use it to avoid making mistakes. This is very important, since even the most minor blots and deviations from the norm can become grounds for the Ministry of Internal Affairs to return documents. This will be a waste of time for you. The application for a foreign passport is located on two pages of one sheet. Two types of filling are allowed: manually or electronically.

A document with blots or corrections will not be accepted for consideration.

Manual filling should be done in block letters only. Black or blue ink should be used. The finished document will contain:

  • personal data;
  • passport details;
  • information about children;
  • information about work.

Options for filling out an application form for an old-style international passport valid for 5 years

Regardless of the fact that a new type of passport has now appeared - biometric passports with a service life of 10 years - Russian citizens continue to issue standard, paper foreign passports for 5 years. To do this, you need to provide the FMS, MFC or through the State Services portal with a list of required documents and fill out a form.

They are very careful about filling out the passport application form correctly. When submitting documents through the MFC, an on-site employee can visually check the application form, or check it in detail with the documents provided (depending on the MFC). The multifunctional center is aimed at helping citizens receive and transfer their documents to the Federal Migration Service, avoiding long queues.

  1. On paper , the application form is submitted in two copies, on one of which the employee marks the acceptance of all documentation. It all depends on the type and complexity of the application - for an adult or a child, a passport to replace the previous one, or one being produced for the first time. In this case, the application form for an old-style international passport is located on the organization’s computer, and the form is filled out directly in front of you.

What does personal data include?

The application form for a foreign passport begins with personal data. You must write your last name, first name and patronymic. No shortcuts are allowed. Everything must correspond to the data in your internal passport. To indicate gender, place the “×” symbol in the desired box. In the computer version, it can be replaced by the letter “x”. Write your date of birth in the standard format: day, month, year.

Next you need to write your place of birth - copy the data from your Russian passport. Information about previous surnames is filled in only by those who changed this data. In addition to the last name, you should write where exactly the change took place and the date of this event. If the surname has changed more than once, an additional sheet is provided in which you should write about each previous surname.

The sixth paragraph of the block with personal data is devoted to registration. You must indicate the location and date of registration.

If you do not live where you are registered, you need to fill out the seventh paragraph, writing in it the address where you live. If we are talking about temporary registration, it is necessary to indicate its duration.

Next is a contact phone number where you can be contacted. If you wish, you can also leave your e-mail in the application. This item is optional.

Sample form for adult citizens

Each citizen, at his own discretion, can fill out the application form manually or on a computer. The application form or completed application is printed on standard A4 sheets on both sides.

How to fill out an application for an old-style international passport for adults:

  1. The individual writes down his/her full name.
  2. Indicates your gender: if male, put the letter “M”, female - “F”.
  3. In numerical form, the day, month, and year of birth are indicated.
  4. According to the information in your internal civil passport, you must enter your place of birth (city, region) into the form.
  5. If the applicant changed his full name, it is necessary to indicate the previous name, place, and date of the change. If this procedure was carried out more than once, then the information is entered into Appendix 2.
  6. Entering the address and date of registration: region, district, city (other locality), street name, building number, house, apartment or private house.
  7. Address of actual residence, if the citizen does not live at the registration address.
  8. Specify the phone number for contacts.
  9. Email address (at the discretion of the applicant).
  10. Information about the internal civil passport: series, number, date of issue of the document, who issued it.
  11. Information is entered about whether the citizen has access to information of special importance or top secret data (institution when the access was issued), obligations under existing contractual agreements, work contracts (name of the company when the documents were signed). This section is completed if such facts exist.
  12. If you have an old international passport, indicate its number, series, date of issue, and who issued the document.

This was the first page of the document, the second begins with a section on the applicant’s employment 10 years before submitting documents for a foreign passport, in which he must display the following information:

  • all organizations in which he worked, educational institutions, if he underwent training, places of service (data is entered on the basis of labor);
  • if the applicant was unemployed for more than a month, he writes that this period “Did not work”, and also indicates where he was registered at that time.

Information on the presence of minor children under 14 years of age is indicated in Appendix 2 (full name, where and when of birth).

At the end of the application, the citizen puts the date of filling out the document and a personal signature. All remaining paragraphs and subparagraphs are filled out by the FMS employee who accepted the document.

Data from internal passport

In the application, in the block dedicated to passport data, you need to enter the number and series of your Russian passport. In this paragraph, special attention must be paid to indicating information about access to information that falls under the category of special importance. We are talking about people whose profession requires them to have access to secret information and, as a result, prohibits or restricts the travel of such categories of people abroad.

If you belong to this category of citizens, you will be denied an old-style passport or a sample with biometric data. It will not be possible to hide the data, since the information is verified. In any case, you will receive a refusal.

Documents required to fill out forms for a foreign passport:

  • Russian passport,
  • international passport (if issued previously),
  • labor activity over the last 10 years, including studies, service in the Army, places of imprisonment (date of admission, dismissal, position, rank (for military personnel), name of the organization (number of military unit, type and branch of the military), legal address,
  • for unemployed and individual entrepreneurs, original work book,
  • for individual entrepreneurs - OGRN and TIN certificates.

For minors:

  • passport of one of the parents (the one who will submit the application),
  • birth certificate/passport of the Russian Federation (from 14 years old),
  • registration certificate (registration),
  • the child’s foreign passport (if issued previously).

List of documents for obtaining a general international passport for adults:

  • Completed application form with unpasted photograph in two copies
  • Passport of a citizen of the Russian Federation and its photocopy (main page and registration)
  • Old passport (if you had one)
  • A certified copy of the work record book or an extract from it. Or the original work book. For individual entrepreneurs - OGRN certificate. By the way, you will need to remember and not forget the dates, addresses, places of work and study over the past 10 years. All this will be reflected in the application.
  • For males aged 18 to 27 years - a military ID with a mark on the completion of military service upon conscription (and its copy from pages 1 to 4, if you served and if you did not serve - a copy of page 1 and page 13) or a corresponding certificate from the military commissariat at your place of residence to confirm information that you are not called up for military service and are not sent to alternative civilian service
  • For women applying for a foreign passport in connection with a change of surname - a copy and original of the marriage certificate
  • Receipt for payment of state duty
  • Photo: 2 pcs for a biometric passport, 4 pcs for a simple passport.

List of documents for obtaining a foreign passport for minors:

  • Completed application form with unpasted photograph in two copies
  • The child’s Russian passport (if available) and its photocopy (main page and registration)
  • Old passport (if you had one)
  • Child's birth certificate and its photocopy
  • An insert for the birth certificate confirming the citizenship of the child (if there is an insert). For some, citizenship is reflected in a stamp on the back of their birth certificate. It also needs to be photocopied.
  • Passport of the child's parent and its photocopy
  • Receipt for payment of state duty, photo
  • Certificate of registration (registration), copy of the document

What to write about work and children

Another section of the form is devoted to work activity. The last 10 years of employment need to be sanctified. Even if you were listed as unemployed, this must be indicated on a special sign. Those who changed jobs many times during the specified period need to write it all down, indicating the dates. Studying also counts. Breaks of more than one month are also necessary.

In the application you need to write not only the names of organizations, but also their addresses. During the unemployed period, you must indicate the address where you lived at the time. If you are a pensioner, you also need to write your home address.

If, while working in one organization, you changed your position one or more times, this fact must be reflected in the application in the corresponding columns of the table. Carefully look at the sample filling to see how it is done correctly.

If you have not written everything yet, and there is no more space on the plate, use the application form No. 2a and write in it everything that does not fit in the main form.

The column about children is filled in if minors do not have their own foreign passport. If the child has documents for traveling abroad, this item can be ignored.

All other columns will be filled out by employees of the Ministry of Internal Affairs. All you have to do is put the date and signature. The latter should not go beyond the boundaries allocated for it. It is also undesirable for it to come into contact with these boundaries.

How many copies do you need?

In accordance with paragraphs. 37.1.1 of the Administrative Regulations, an application for the issuance of an old-style international passport to a citizen over 18 years of age is submitted in two copies . Accordingly, there should also be two attachments No. 3 to the application for a foreign passport.
